I warrant that I have authority to sign this document and accept personal responsibility for the accuracy of all information supplied. I acknowledge and understand that all information supplied by Perisher is to be used for the sole purpose of the story/article, as described above, and that I will use only expressly approved and authorised images and information supplied by Perisher. I agree that I will send to Perisher the completed story/article. I confirm that upon being granted approval to visit Perisher as part of the Visiting Journalist Program, I will complete a Visiting Media Waiver and sign in with Perisher Marketing upon arrival.
